Terms and Conditions
At Evergreen Med Spa & Wellness, we reserve dedicated time exclusively for you. To respect the schedules of our providers and guests, we kindly ask that you review our policy below.
24-Hour Cancellation Policy
We require at least 24 hours’ notice to cancel or reschedule an appointment.
Cancellations made with less than 24 hours’ notice will incur:

$50 late cancellation fee
No-Show Policy
Failure to show for a scheduled appointment without notice will result in:

100% charge of the scheduled service
OR
-
Deduction of the service from package or membership credits
Future appointments may require prepayment.
Late Arrivals
Guests arriving more than 10–15 minutes late may:

Have treatment time shortened

Be rescheduled

Be subject to the applicable cancellation fee if rescheduling is required
Deposits & Prepayments
Select services (including injectables, RF Microneedling, PRP, and body contouring) may require a non-refundable deposit at booking.
Deposits are applied toward your service total but are forfeited for late cancellations or no-shows.
Emergencies
We understand unexpected situations arise. Fees may be waived at management’s discretion in cases of verified illness or emergency.

If you consent to receive text messages from Evergreen Med Spa & Wellness, you may receive messages related to:

Promotions or special offers
Example Message:
“Hello, this is a friendly reminder of your upcoming appointment with Evergreen Med Spa & Wellness at 4035 Evergreen Village Square, Suite 60, San Jose, CA 95135 on [Date] at [Time]. You can reply STOP to opt out of SMS messaging at any time.”
