Membership Policy & Terms Agreement

Thank you for joining Evergreen Med Spa & Wellness. Our membership program is designed to provide consistent results, preferred pricing, and exclusive benefits for our valued members.

By enrolling, you agree to the following terms:

1. Membership Commitment​

All memberships require a 3-month minimum commitment.

After the initial 3-month term, membership continues on a month-to-month basis unless canceled in writing with 30 days’ notice.

Early cancellation before completion of the 3-month minimum will require payment of the remaining committed months

2. Monthly Billing

  • Membership fees are billed each month automatically to the card on file.

  • Billing occurs on the same date each month based on enrollment date.

Declined payments must be resolved within 5 business days to avoid suspension of membership benefits.

3. Monthly Service Credits

  • One designated monthly treatment is included per membership tier.

  • Unused monthly credits may roll over for up to 60 days from the date of issue.

  • Credits expire after 60 days and do not carry monetary value.

  • Credits are non-transferable and may only be used by the enrolled member.

4. Member Pricing & Discounts

  • Member pricing and per-unit injectable discounts apply only while membership is active and in good standing. 

  • Discounts cannot be combined with other promotions unless explicitly stated.

  • Retail discounts apply to in-stock items only.

​Evergreen Med Spa reserves the right to modify standard member pricing with 30 days’ notice.

5. Cancellation & No-Show Policy

​Membership appointments are subject to the Evergreen Med Spa Cancellation Policy:

  • $50 late cancellation fee (under 24 hours)

  • No-shows may result in forfeiture of the monthly credit.

Repeated late cancellations (2 or more within 6 months) may result in suspension of membership privileges at management discretion.

6. Freezes & Holds

​Memberships may be frozen:

  • Maximum of 1 freeze per calendar year

  • Freeze duration: up to 30 days

  • Freeze requests must be submitted in writing at least 7 days before billing.

Freezes are not permitted during the initial 3-month commitment period.

7. Refund Policy​​

Membership fees are non-refundable once billed.

Unused services, expired credits, or partially used months are not eligible for refund or cash value.

8. Medical Eligibility​​

All services are subject to provider evaluation and medical appropriateness.

Membership does not guarantee eligibility for specific treatments.

9. Transfer & Sharing​

​Membership benefits are non-transferable and may not be shared unless otherwise specified in writing.

10. Founding Member Status (Limited to First 100 Members)

Evergreen Med Spa & Wellness may offer a Founding Member promotion to the first 100 enrolled members.

 

Founding Members receive: 

​

A lifetime discount on eligible services and retail, while membership remains active and in good standing.

 

Founding Member benefits: 

  • Apply only while membership remains uninterrupted. 

  • Will be forfeited if membership is canceled or lapses due to non-payment. 

  • Are non-transferable.

  • Do not apply to injectable units already discounted under membership pricing, promotional packages, or medical weight loss medications unless specifically stated.

Founding Member status cannot be reinstated once lost.

11. Modification & Termination

Evergreen Med Spa & Wellness reserves the right to:

  • Modify membership terms with written notice

  • Adjust pricing for new members

  • Suspend or terminate membership for policy abuse, inappropriate conduct, or repeated non-compliance

Agreement & Authorization

By enrolling in an Evergreen Med Spa & Wellness Membership, you authorize automatic recurring billing to your card on file and agree to all membership terms outlined above.
